There’s nothing quite like March Madness. The brackets, the buzzer beaters, the upsets nobody saw coming; it’s the most exciting month in basketball. It has a way of pulling everyone back onto the court. Weekend warriors dust off their sneakers. High school players push harder in hopes of a standout spring season. Pickup games that were on pause all winter suddenly fill up again. 

All of that is great, right up until someone lands wrong on an ankle, takes a hard cut and feels something pop in their knee, or jams a finger on a ball they had no business going for. When enthusiasm is high and conditioning is low, it is prime time for orthopedic injuries. Here’s what we see most and what you should do if it happens to you. 

The Most Common Basketball Injuries We Treat

Ankle Sprains

The most common basketball injury, full stop. They happen on landings, on cuts, on contact, and they happen fast. A mild sprain might feel walkable in the moment, which is exactly why so many players shake it off and keep going. That’s usually a mistake. Playing through an unexamined sprain can turn a Grade 1 injury into a Grade 3, or mask a more serious fracture. If your ankle swells, bruises, or won’t bear weight comfortably within a few hours, get it looked at. 

ACL and Meniscus Tears

The sounds players dread most. ACL tears typically happen on deceleration, planting a foot and changing direction, and the pop is usually unmistakable. Meniscus tears can happen the same way, or from a deep squat or twist under load. Both injuries are more common in players whose legs aren’t fully conditioned for the lateral demands of basketball, which is exactly the profile of someone who just came back to the game for March Madness pickup runs. 

Patellar Tendinitis (Jumper’s Knee)

High school players and competitive recreational athletes are especially vulnerable to patellar tendinitis which is inflammation of the tendon that connects the kneecap to the shinbone. It builds gradually from the repetitive jumping and sprinting demands of basketball, and it has a way of going from a minor nuisance to a real problem over the course of a season. The earlier it’s addressed, the easier it is to manage. 

Jammed and Fractured Fingers

Finger injuries are the most dismissed injuries in basketball and some of the most undertreated. A “jammed finger” that doesn’t improve within a few days could be a ligament injury, a fracture,

or a dislocation that needs to be properly set. Left untreated, finger injuries can lead to chronic instability, stiffness, and long-term limitations. Our hand specialists see these regularly and can get you diagnosed and splinted the same day. 

The difference between a minor setback and a months-long recovery often comes down to one thing: how quickly you get evaluated. 

When to Come In — and When to Come In Now

Not every basketball injury needs an emergency visit. But some do, and it’s worth knowing the difference.

  • A pop or snap in the knee, ankle, or finger followed by swelling 
  • Inability to bear weight on an ankle or knee after 15–20 minutes 
  • Significant swelling or bruising in any joint within the first hour

  • Knee pain that locks, gives way, or prevents normal movement
